    Validação da coerência estrutural do Questionário de Saúde Geral

    OBJECTIVE: Investigate the structural coherency of the 60-item version of the General Health Questionnaire via exploratory and confirmatory factor analyses. METHOD: The study design is a cross-sectional survey. A random sample of 146 individuals from the city of Divinópolis-MG volunteered to participate in the present study and responded to the 60-item version of the General Health Questionnaire adapted and validated for use in Brazil. Statistics consisted of exploratory and confirmatory factor analysis. Reliability was estimated using Cronbach's alpha method. RESULTS: Alpha coefficients for all five content scales of the General Health Questionnaire were high (α > 0.8). For four of the five scales, a unifactorial model of constituent items provided a good fit to the data. Items comprising the fifth scale, Psychic Stress, exhibited a two-correlated factor structure. A factor analysis of scores for the five scales yielded strong evidence of coherency, with all scales loading substantially on a single common factor. CONCLUSION: The General Health Questionnaire shows good psychometric coherency as evidenced by high internal consistency and unidimensionality of all but one of its constituent scales, and uniformly high loadings of all scales on a single overarching factor. These results are consistent with prior findings from the General Health Questionnaire developmental study and Brazilian adaptation studies.OBJETIVO: Investigar a coerência estrutural da versão de 60 itens do Questionário de Saúde Geral de Goldberg por meio da aplicação de análises fatoriais exploratórias e confirmatórias. MÉTODO: Trata-se de um estudo de levantamento de desenho transversal. Uma amostra aleatória de 146 indivíduos voluntários da cidade de Divinópolis-MG participou do presente estudo respondendo à versão de 60 itens do Questionário de Saúde Geral. As estatísticas consistiram de análises fatoriais exploratórias e confirmatórias. Fidedignidade foi estimada pelo método alpha de Cronbach. RESULTADOS: Os coeficientes alpha para todas as cinco escalas do Questionário de Saúde Geral foram elevados (α > 0,8). Para quatro das cinco escalas, um modelo unifatorial com base em seus respectivos itens apresentou bom ajuste aos dados. Itens que compunham a quinta escala, Estresse Psíquico, exibiram uma estrutura de dois fatores correlacionados. Uma análise fatorial dos escores das cinco escalas produziu fortes evidências de coerência, com todas as escalas apresentando cargas fatoriais robustas em um único fator. CONCLUSÃO: O Questionário de Saúde Geral apresenta bons indicadores psicométricos de coerência deflagrados pela elevada consistência interna e unidimensionalidade de todas menos uma de suas escalas constituintes e pelas cargas fatoriais, univocamente elevadas, em uma única e inclusiva dimensão.     Linguistic features of the therapeutic alliance in the first session: a psychotherapy process study

    Critical aspects of the therapeutic alliance appear to be established as early as the first session. Specifically, the affective bond between the therapeutic dyad appears to develop early in treatment and tends to remain stable over time, while agreements on goals and tasks tend to fluctuate over the course of treatment. Are there distinguishable early signs of a strong therapeutic alliance? In this study, we examined how some linguistic measures indicative of joint emotional elaboration correlated with a measure of the therapeutic alliance assessed within a single session. Initial intake sessions with 40 patients with varying diagnoses were videotaped, transcribed, and analyzed using linguistic measures of referential process and then scored with the Segmented Working Alliance Inventory-Observer form. Results showed that patients who were rated as more emotionally engaged in relating their experiences and then reflecting on them by mid-session also had higher scores in the therapeutic alliance by the final part of that same session. An implication of this study is that the interpersonal factors facilitating elaboration of inner experience, including elements of warmth, safety, and analytic trust, are related to the development of early therapeutic alliance. These findings did not appear to be dependent on the patient’s psychopathology. This study is one in a growing line of research exploring how patients speak rather than just the content of what they say

    Biomechanical, biochemical, and morphological mechanisms of heat shock-mediated germination in Carica papaya seed.

    Carica papaya (papaya) seed germinate readily fresh from the fruit, but desiccation induces a dormant state. Dormancy can be released by exposure of the hydrated seed to a pulse of elevated temperature, typical of that encountered in its tropical habitat. Carica papaya is one of only a few species known to germinate in response to heat shock (HS) and we know little of the mechanisms that control germination in tropical ecosystems. Here we investigate the mechanisms that mediate HS-induced stimulation of germination in pre-dried and re-imbibed papaya seed. Exogenous gibberellic acid (GA3 ≥250 µM) overcame the requirement for HS to initiate germination. However, HS did not sensitise seeds to GA3, indicative that it may act independently of GA biosynthesis. Seed coat removal also overcame desiccation-imposed dormancy, indicative that resistance to radicle emergence is coat-imposed. Morphological and biomechanical studies identified that neither desiccation nor HS alter the physical structure or the mechanical strength of the seed coat. However, cycloheximide prevented both seed coat weakening and germination, implicating a requirement for de novo protein synthesis in both processes. The germination antagonist abscisic acid prevented radicle emergence but had no effect on papaya seed coat weakening. Desiccation therefore appears to reduce embryo growth potential, which is reversed by HS, without physically altering the mechanical properties of the seed coat. The ability to germinate in response to a HS may confer a competitive advantage to C. papaya, an opportunistic pioneer species, through detection of canopy removal in tropical forests

    Evidence of multidecadal salinity variability in the eastern tropical North Atlantic

Although adequate instrumental sea-surface temperature (SST) records exist for most of the surface oceans over the past 100-150 years, records of salinity really only exist for the last 40-50 years. Here we show that longer proxy records from corals (Siderastrea radians) in the eastern tropical North Atlantic are dominated by multi-decadal variations in salinity which are correlated with the relationship between SST and the North Atlantic Oscillation (NAO) over the course of the 20th century. The data reveal an increase in eastern tropical North Atlantic salinity of +0.5 psu between about 1950-1990. Rather than a monotonic secular increase, as indicated by some instrumental records, the pre-instrumental coral proxy records presented here suggest that salinity in the tropical North Atlantic is periodic on a decadal to multi-decadal scale

    The in vitro toxicology of Swedish snus

    Three commercial brands of Swedish snus (SWS), an experimental SWS, and the 2S3 reference moist snuff were each tested in four in vitro toxicology assays. These assays were: Salmonella reverse mutation, mouse lymphoma, in vitro micronucleus, and cytotoxicity. Water extractions of each of the 5 products were tested using several different concentrations; the experimental SWS was also extracted using dimethyl sulfoxide (DMSO). Extraction procedures were verified by nicotine determinations. Results for SWS in the mutagenicity assays were broadly negative: there were occasional positive responses, but these were effectively at the highest concentration only (concentrations well above those suggested by regulatory guidelines), and were often associated with cytotoxicity. The 2S3 reference was unequivocally positive in one of the three conditions of the micronucleus assay (MNA), at the highest concentration only. Positive controls produced the expected responses in each assay. The SWS data are contrasted with data reported for combusted tobacco in the form of cigarettes, where strongly positive responses have been routinely reported for mutagenicity and cytotoxicity. These negative findings in a laboratory setting concur with the large amount of epidemiological data from Sweden, data showing that SWS are associated with considerably lower carcinogenic potential when compared with cigarettes

    Identification of Nine Novel Loci Associated with White Blood Cell Subtypes in a Japanese Population

    White blood cells (WBCs) mediate immune systems and consist of various subtypes with distinct roles. Elucidation of the mechanism that regulates the counts of the WBC subtypes would provide useful insights into both the etiology of the immune system and disease pathogenesis. In this study, we report results of genome-wide association studies (GWAS) and a replication study for the counts of the 5 main WBC subtypes (neutrophils, lymphocytes, monocytes, basophils, and eosinophils) using 14,792 Japanese subjects enrolled in the BioBank Japan Project. We identified 12 significantly associated loci that satisfied the genome-wide significance threshold of P<5.0×10−8, of which 9 loci were novel (the CDK6 locus for the neutrophil count; the ITGA4, MLZE, STXBP6 loci, and the MHC region for the monocyte count; the SLC45A3-NUCKS1, GATA2, NAALAD2, ERG loci for the basophil count). We further evaluated associations in the identified loci using 15,600 subjects from Caucasian populations. These WBC subtype-related loci demonstrated a variety of patterns of pleiotropic associations within the WBC subtypes, or with total WBC count, platelet count, or red blood cell-related traits (n = 30,454), which suggests unique and common functional roles of these loci in the processes of hematopoiesis. This study should contribute to the understanding of the genetic backgrounds of the WBC subtypes and hematological traits

    Soluble iron nutrients in Saharan dust over the central Amazon rainforest

    The intercontinental transport of aerosols from the Sahara desert plays a significant role in nutrient cycles in the Amazon rainforest, since it carries many types of minerals to these otherwise low-fertility lands. Iron is one of the micronutrients essential for plant growth, and its long-range transport might be an important source for the iron-limited Amazon rainforest. This study assesses the bioavailability of iron Fe(II) and Fe(III) in the particulate matter over the Amazon forest, which was transported from the Sahara desert (for the sake of our discussion, this term also includes the Sahel region). The sampling campaign was carried out above and below the forest canopy at the ATTO site (Amazon Tall Tower Observatory), a near-pristine area in the central Amazon Basin, from March to April 2015. Measurements reached peak concentrations for soluble Fe(III) (48 ng m&minus;3), Fe(II) (16 ng m&minus;3), Na (470 ng m&minus;3), Ca (194 ng m&minus;3), K (65 ng m&minus;3), and Mg (89 ng m&minus;3) during a time period of dust transport from the Sahara, as confirmed by ground-based and satellite remote sensing data and air mass backward trajectories. Dust sampled above the Amazon canopy included primary biological aerosols and other coarse particles up to 12 &micro;m in diameter. Atmospheric transport of weathered Saharan dust, followed by surface deposition, resulted in substantial iron bioavailability across the rainforest canopy. The seasonal deposition of dust, rich in soluble iron, and other minerals is likely to assist both bacteria and fungi within the topsoil and on canopy surfaces, and especially benefit highly bioabsorbent species. In this scenario, Saharan dust can provide essential macronutrients and micronutrients to plant roots, and also directly to plant leaves. The influence of this input on the ecology of the forest canopy and topsoil is discussed, and we argue that this influence would likely be different from that of nutrients from the weathered Amazon bedrock, which otherwise provides the main source of soluble mineral nutrients

    Evaluation of Bioelectrical Impedance Analysis for Identifying Overweight Individuals at Increased Cardiometabolic Risk:A Cross-Sectional Study

    OBJECTIVE: To investigate whether bioelectrical impedance analysis could be used to identify overweight individuals at increased cardiometabolic risk, defined as the presence of metabolic syndrome and/or diabetes.DESIGN AND METHODS: Cross-sectional study of a Scottish population including 1210 women and 788 men. The diagnostic performance of thresholds of percentage body fat measured by bioelectrical impedance analysis to identify people at increased cardiometabolic risk was assessed using receiver-operating characteristic curves. Odds ratios for increased cardiometabolic risk in body mass index categories associated with values above compared to below sex-specific percentage body fat thresholds with optimal diagnostic performance were calculated using multivariable logistic regression analyses. The validity of bioelectrical impedance analysis to measure percentage body fat in this population was tested by examining agreement between bioelectrical impedance analysis and dual-energy X-ray absorptiometry in a subgroup of individuals.RESULTS: Participants were aged 16-91 years and the optimal bioelectrical impedance analysis cut-points for percentage body fat for identifying people at increased cardiometabolic risk were 25.9% for men and 37.1% for women. Stratifying by these percentage body fat cut-points, the prevalence of increased cardiometabolic risk was 48% and 38% above the threshold and 24% and 19% below these thresholds for men and women, respectively. By comparison, stratifying by percentage body fat category had little impact on identifying increased cardiometabolic risk in normal weight and obese individuals. Fully adjusted odds ratios of being at increased cardiometabolic risk among overweight people with percentage body fat ?25.9/37.1% compared with percentage body fat &lt;25.9/37.1% as a reference were 1.93 (95% confidence interval: 1.20-3.10) for men and 1.79 (1.10-2.92) for women.CONCLUSION: Percentage body fat measured using bioelectrical impedance analysis above a sex-specific threshold could be used in overweight people to identify individuals at increased cardiometabolic risk, who could benefit from risk factor management
